1、Increasing the dose of aclarubicin in low-dose cytarabine and aclarubicin in combination with granulocyte colony-stimulating factor (CAG regimen) can safely and effectively treat relapsed or refractory acute myeloid leukemia

摘要:It is difficult for relapsed and refractory acute myeloid leukemia (AML) patients to achieve complete remission (CR). The CAG regimen [low-dose cytarabine and aclarubicin in combination with granulocyte colony-stimulating factor (G-CSF)] has been used to treat relapsed and refractory AML patients, and showed good therapeutic efficacy. It is unknown, however, whether increasing the dose of aclarubicin in CAG regimen could treat relapsed or refractory AML safely and effectively. We evaluate the efficacy and tolerability of increasing the dose of aclarubicin in CAG regimen, in 37 relapsed or refractory AML patients. All patients were treated with CAG regimen including low-dose cytarabine (10 mg/m2 every 12 h, days 1-14), aclarubicin (5-7 mg/m2 every day, days 1-14), and G-CSF (200 μg/m2 every day, days 1-14) priming. After a single course of therapy, the overall response [CR + partial remission (PR)] rate of all patients was 78.4 % (29/37), in which the CR rate was 62.2 % (23/37). There was no early death. The median overall survival was 6 months (range 2-36 months). Myelosuppression was ubiquitous, but tolerated. No severe non-hematologic toxicity was observed. Thus, increasing the dose of aclarubicin in CAG regimen can be used safely and effectively in the treatment of relapsed or refractory AML.

摘要:• Efficacy of CAG and Hyper-CVAD were similar for relapsed/refractory Ph−-T-ALL. • Efficacy of CAG group for relapsed/refractory Ph−-B-ALL was lower than Hyper-CVAD. • HD-CAG could not improve efficacy than CAG. • CAG had lesser adverse event than Hyper-CVAD.We treated 90 relapsed/refractory Philadelphia chromosome-negative acute lymphoblastic leukemia (Ph−-ALL) patients with CAG regimen [cytarabine (10 mg/m2/12 h, days 1-14), aclarubicin (5-7 mg/m2/day, days 1-8), granulocyte colony-stimulating factor (200 μg/m2/day, days 1-14)], 82 relapsed/refractory Ph−-ALL patients were treated with increasing aclarubicin dose CAG (5-7 mg/m2/day, days 1-14, HD-CAG). 96 relapsed/refractory Ph−-ALL patients treated with Hyper-CVAD regimen (control group). After one therapy course, among all groups, there were no statistically significant differences with complete remission (CR) and overall response [OR, CR + partial remission (PR)] rates (P > 0.05). In CAG group, CR and OR rates for T-ALL exceeded those for B-ALL (P = 0.001, 0.007), while in HD-CAG and control groups, those were not statistically significantly different (P > 0.05). CR and OR rates of CAG group for B-ALL were lower than control group (P = 0.004, 0.012). Among all groups, there were no statistically significant differences with CR and OR rates for T-ALL (P > 0.05). CAG had lesser adverse event than Hyper-CVAD. The overall survival at 3 years for all groups were similar. Efficacy of CAG regimen was similar in comparison to Hyper-CVAD for relapsed/refractory Ph−-T-ALL. HD-CAG could not improve efficacy than CAG regimen.

摘要:• Efficacy of HD-CAG for relapsed/refractory MPAL exceeded DOAP regimen. • Efficacy of HD-CAG group for relapsed/refractory B + My MPAL was lower than T + My MPAL. • Efficacy of HD-CAG for relapsed/refractory MPAL, AML, and ALL were similar. • Side effects were similar between HD-CAG and DOAP regimen.We treated 60 relapsed/refractory mixed-phenotype acute leukemia patients (MPAL-1) with increasing the aclarubicin dose in CAG regimen (HD-CAG, cytarabine (10 mg/m2/12 h, days 1-14), aclarubicin (5-7 mg/m2/day, days 1-14), granulocyte colony-stimulating factor (200 μg/m2/day, days 1-14). This was compared to 64 relapsed/refractory MPAL patients (MPAL-2) treated with DOAP regimen (daunorubicin, vincristine/vindesine, cytarabine and prednisone), 113 relapsed/refractory acute myeloid leukemia (AML) patients and 78 acute lymphocytic leukemia (ALL) patients treated with HD-CAG regimen. After one course, complete remission (CR) and overall response [OR, CR + partial remission (PR)] rates for MPAL-1 exceeded MPAL-2 (CR, 61.02% vs. 28.13%, P = 0.000; OR, 72.88% vs. 34.38%, P = 0.000), but these data were similar to AML and ALL (P > 0.05). In MPAL-1 group, CR and OR rates of T-lymphoid + myeloid immunophenotype were higher than B-lymphoid + myeloid immunophenotype (CR, 81.82% vs. 44.12%, P = 0.005; OR, 90.91% vs. 58.82%, P = 0.009). The overall survival at 3 years in MPAL-1, MPAL-2, AML and ALL groups were 14.2% ± 6.8%, 14.1% ± 6.4%, 17.3% ± 5.0% and 15.0% ± 5.3% (P > 0.05). Side effects were similar between HD-CAG and DOAP (P > 0.05). HD-CAG regimen is efficacious for relapsed/refractory MPAL, especially for T + My patients.

6、Photodynamic therapy for symptomatic circumscribed choroidal hemangioma in 22 Chinese patients: A retrospective study

摘要:• PDT for 22 CCH in Chinese patients. • Multiple images follow up for 12 months. • BRAO due to PDT on CCH.The purpose of this study is to describe the effects and complications of photodynamic therapy (PDT) on Chinese patients with circumscribed choroidal hemangioma (CCH). In this retrospective study, 22 CCH patients who underwent PDT performed 15 min after the injection of intravenous verteporfin (6 mg/m2)with multiple 83-second laser spots at 689 nm (50 J/cm2) were studied. Fluorescein angiography and/or indocyanine green angiography, B-scan ultrasonography and optical coherence tomography were performed in all patients. Follow-up was performed until 12 months post-treatment. All patients were treated with one session, except 1 case with prior transpupillary thermotherapy history. At the 12-month follow-up, the mean of the best corrected visual acuity (BCVA) increased from 0.40 ± 0.38 to 0.56 ± 0.42 (p ), tumors became thinner (1.96 ± 2.65 mm vs. 4.31 ± 2.04 mm) (p ), and exudative detachment were diminished. The mean fovea center thickness (FCT) decreased from 540.1 ± 470.6 to 171.6 ± 79.3 μm at the 3-month follow-up. The 12-month BCVA was correlated with prior laser treatment, symptom duration, baseline CCH diameter and thickness, baseline FCT and cystoid macular edema. One patient developed a branch retinal artery occlusion. In conclusion, PDT is an effective and safe treatment for CCH. Specific PDT protocols for CCH should be standardized. The retinal arteriole should be spared during the treatment.

7、Efficacy of porcine antihuman lymphocyte immunoglobulin compared to rabbit antithymocyte immunoglobulin as a first-line treatment against acquired severe aplastic anemia

摘要:Immunosuppressive therapy (IST) with antithymocyte immunoglobulin (ATG) or antilymphocyte immunoglobulin (ALG) and cyclosporine A (CsA) is the treatment of choice against severe aplastic anemia (SAA) worldwide. However, a comparison of the efficacy of porcine ALG (pALG) and rabbit ATG (rATG) as a first-line treatment for acquired SAA has not been reported. In the present study, we retrospectively analyzed SAA patients treated with either pALG (n = 43) or rATG (n = 32) and compared their hematologic responses and survivals. There were no significant differences in overall response (OR) rates between pALG and rATG groups at 3 months (OR 41.86 versus 40.62 % (P = 0.914), 6 months (OR 66.67 versus 61.29 % (P = 0.635), 9 months (OR 69.05 versus 61.29 % (P = 0.490), or 12 months (OR 69.05 versus 64.51 % (P = 0.684), respectively. The OR rates in patients with SAA or very severe aplastic anemia (vSAA) in both groups were similar after a 12-month treatment (pALG 74.07 versus 60.00 %, P = 0.550; rATG 70.00 versus 54.55 %, P = 0.640). Patients who experienced P = 0.021; rATG 87.50 versus 40.00 %, P = 0.006). There were no significant differences in 2-year overall survival (OS) between pALG (87.4 ± 6.2 %) and rATG (83.2 ± 7.8 %) (P = 0.493). Infection was the major cause of death in both groups. In summary, pALG + CsA showed similar efficacy as rATG + CsA, as a first-line treatment for acquired SAA.

8、Design Strategy for the Multilayer Core-Shell Bioceramics with Controlled Chemistry

摘要:New strategies for fabricating multiphase bioceramic porous scaffolds with time-dependent biodegradation and pore network enlargement are of fundamental importance in the advancement of bioceramics. Here, we developed a one-step preparation of core-shell bioceramic microspheres (~2 mm in diameter) with single- or double-shell structure through a coaxially aligned multilayer capillary system. The Ca-phosphate (CaP) and Ca-silicate (CaSi) ceramic phase distribution could be also adjusted by extruding through different capillaries, and thus the biodegradation rate would be readily tailored over time. When the polystyrene (PS) microbeads of ~15 μm in diameter were premixed into the CaP- or CaSi-containing alginate slurry, the tailorable porous structures could be introduced into the core or different shell layers of the microspheres. These micropores may potentially maximize the permeability for rapid exchange of guest molecules or inorganic ions from the bioceramics. Totally, such strategy is promising because the ceramic phases with different biological properties can be assembled into the core-shell bioceramic microspheres, and thus the macropore structure evolution may be readily manipulated in the closely packed microsphere systems. We believe our gradient hybrid methodology will have potential in various categories of advanced biomaterials of organic-inorganic composites.

9、Relation between microPET imaging and rotational behavior in a parkinsonian rat model induced by medial forebrain bundle axotomy

摘要:• Apomorphine-induced rotations increased after medial forebrain bundle (MFB) axotomy. • [11C]CFT uptake ratio decreased after MFB axotomy. • [11C]raclopride uptake ratio increased after MFB axotomy. • [11C]CFT and [11C]raclopride uptake ratio correlated with rotational behavior.The purpose of the current study was to examine the relation between apomorphine (APO) induced rotational behavior and the pre- and post-synaptic dopaminergic function in a parkinsonian rat model induced by medial forebrain bundle (MFB) axotomy. The brains of these rats were unilaterally lesioned by mechanical transection of the nigrostriatal dopamine pathway at the MFB. Behavioral studies were carried out by APO challenge prior to and 1, 3, and 5 weeks after MFB axotomy. MicroPET scans with [11C]CFT and [11C]raclopride were performed 2 days after the behavioral test. The two PET scans were separated by an interval of 24-48 h. Immunohistochemistry was conducted 4 days after the last PET scan. Our data showed that [11C]CFT binding decreased progressively 1, 3, and 5 weeks postlesion, and there was a significant nonlinear correlation between [11C]CFT uptake ratio (right/left) and APO induced rotations. In contrast, [11C]raclopride binding only increased significantly 3 weeks postlesion, and there was a positive linear correlation between [11C]raclopride uptake ratio (right/left) and APO induced rotations. Postmortem immunohistochemical studies confirmed the loss of both striatal dopamine fibers and nigral neurons on the lesioned side. These findings not only demonstrate the relation between APO induced rotational behavior and the pre- and post-synaptic dopamine function but also indicate the utility and validity of in vivo PET imaging in understanding disease mechanisms and progression, which should in turn lead to development of new therapies.

10、Influence of valproic acid concentration and polymorphism of UGT1A4*3, UGT2B7 -161C > T and UGT2B7*2 on serum concentration of lamotrigine in Chinese epileptic children

摘要:To investigate the impact of valproic acid (VPA) and genetic polymorphism of the major metabolizing enzyme (UGT1A4, UGT2B7) of lamotrigine (LTG) and VPA on LTG concentration in Chinese epileptic children.Three single nucleotide polymorphisms (UGT1A4*3, UGT2B7 -161C > T and UGT2B7*2) were analyzed by polymerase chain reaction-restriction fragment length polymorphism or direct DNA sequencing. The concentrations of LTG and VPA were measured by high-performance liquid chromatography (HPLC) and fluorescence polarization immunoassay, respectively. The adjusted concentration of LTG was defined as the concentration-to-dose-ratio (CDRLTG). Data analysis was performed using IBM SPSS Statistics 21.0.A total of 56 patients treated with LTG as monotherapy and 158 patients treated with LTG plus VPA were included in this study. In the polytherapy group, LTG concentration showed a good linear relationship with gender, age, daily LTG dose, VPA concentration, and UGT1A4*3 polymorphism, but had no relationship with the polymorphism of UGT2B7 -161C > T or UGT2B7*2. Moreover, LTG concentration and CDRLTG for the UGT1A4*3 were higher compared to UGT1A4*1 (LTG: 7.24 ± 3.51 vs 5.26 ± 3.27 μg/mL, p = 0.001; CDRLTG: 2.75 ± 1.02 vs 2.14 ± 0.96 μg/mL per mg/kg, p LTG. The patients in the polytherapy group were divided into two subgroups according to VPA concentration (lower/higher: 10-50/50-125 μg/mL). CDRLTG values of the patients carrying the UGT1A4*3 genotype were higher compared to UGT1A4*1*1 (2.86 ± 1.03 vs 2.22 ± 0.94 μg/mL per mg/kg, p = 0.001) only when the VPA concentration was higher.UGT1A4*3 polymorphism had an effect on LTG concentration only with VPA co-administration, and the effect was remarkable when VPA concentration was higher.

11、Evaluation of nigrostriatal damage and its change over weeks in a rat model of Parkinson's disease: small animal positron emission tomography studies with [11C]β-CFT

摘要:Introduction: The cardinal pathological feature of Parkinson's disease (PD) is progressive loss of dopaminergic neurons. Since dopamine transporter (DAT) is a protein located presynaptically on dopaminergic nerve terminals, radioligands that bind to these sites are promising radiopharmaceuticals for evaluation of the integrity of the dopamine system. This study using positron emission tomography (PET) tracers, [11C]-2β-carbomethoxy-3β-(4-fluorophenyl)-tropane ([11C]β-CFT, radioligand for DAT), was aimed at evaluating the degree of nigrostriatal damage and its change over weeks in a rat model of PD.Methods: The brains of these rats were unilaterally lesioned by mechanical transection of the nigrostriatal dopamine pathway at the medial forebrain bundle (MFB). Behavioral studies were carried out by apomorphine (APO) challenge prior to and 1, 2 and 4 weeks after MFB axotomy. Small animal PET scans were performed 2 days after the behavioral test. Immunohistochemistry was conducted 4 days after the last PET scan.Results: Compared with the contralateral intact side, a progressively decreased [11C]β-CFT binding was observed on the lesioned side which correlated inversely with the APO-induced rotations. Postmortem immunohistochemical studies confirmed the loss of both striatal dopamine fibers and nigral neurons on the lesioned side.Conclusion: These findings not only demonstrate that the neuronal degeneration in this model is relatively slow, but also suggest [11C]β-CFT is a sensitive marker to monitor the degree of nigrostriatal damage and its change over weeks. This marker can be used prospectively to study the progression of the disease, thereby making detection of early phases of PD possible.
